An intelligent recursive algorithm computes information concerning the relative position of the vertebral bodies of the thoracic and lumbar spine, while taking into account the local curvature (kyphotic or lordotic). Accurate spinal segmental motion of all vertebral bodies is obtained as the final result. METHOD FOR RECORDING THE SPINE DURING PHYSICAL EXAMINATION
Where, when and why was the Idiag M360 system created?
The idea of a device for recording the surface of the spine was influenced by a study performed and published in 1994 by Dr. Seichert and Prof. Senn (Clinic of Physical Medicine and Rehabilitation of the Ludwig-Maximilian University of Munich, Germany). In cooperation with developers from our Swiss partner company, idiag AG, they developed the first prototype of the SpinalMouse (Idiag M360). In 1999 idiag AG launched the first Idiag M360 devices on the Swiss market. Development was mainly motivated by the fact that there had previously been no methodology or device that allowed a non-invasive, radiation-free, simple and fast – but still reliable and cost effective – assessment of the shape and segmental mobility of the spine.
What innovations does the Idiag M360 bring for diagnosis and evaluation?
The Idiag M360 provides an accurate, simple, fast and cost-effective assessment procedure. It allows recording of the spine without the use of radiation or chemicals; this means that there are no harmful side effects. The Idiag M360 can be used on the same patient as often or as frequently as desired. However, it must be emphasized, that the Idiag M360 technology primarily addresses the postural and functional aspects of the spine, derived from superficial examination. The Idiag M360 is not an expert system per se; it gives indications, but it requires a medical expert to formulate diagnoses. In contrast, X-rays, CT scans and MRIs mainly yield structural information thanks to their tissue and bone penetrating technology and are therefore essential for analysing fractures or degenerative diseases, among others.

Could someone without specific back pain benefit from an Idiag M360 assessment?
The Idiag M360 can provide useful information from so-called “screenings” or “wellness checks”. Spine and back related pain and disorders usually have multi-factorial causes and may develop in nebulous and creeping ways. Regular screenings help to establish baseline information and may alert a person to changes. Bringing such information to the attention of that person’s physician can be extremely helpful in preventing further problems or allowing early diagnosis of existing ones.
HOW IT WORKS
What does the Idiag M360 do?
The Idiag M360 is a computer-assisted medical device that can be used to determine the shape and mobility of the spinal column by simply gliding the device manually down the back. There is a strong relationship between the surface line on which the recording is done and the midline of the spine in terms of alignment, range of motion, performance and its functions. The Idiag M360 device has two wheels arranged on a mobile support that allows the contour of the spine to be tracked. This shape is recorded by sensors that transmit the data to a computer program through a Bluetooth connection.
From the superficial shape, an intelligent recursive algorithm computes information concerning the relative position of the vertebral bodies of the thoracic and lumbar spine, while taking into account the local curvature (kyphotic or lordotic). The final result is an accurate segmental localization of all vertebral bodies as the projection of their midpoints on the superficial contour of the back.

What is the operating principle?
The equipment records the distance plus the Idiag M360’s position in the x, y and z planes at any time during the assessment. This information, combined with known anthropometric data about vertebral body dimensions, is used to calculate the relative positions of the sacrum, the vertebral bodies and the angles between them using a complex algorithm.
Does the Idiag M360 detect the Cobb angle as well?
The values generated by the Idiag M360 software show the angles between lines going through the midplane of the vertebral bodies (see below). Whereas the Cobb angle detects angles with the end plates as reference points. So whilst the measuring points on the vertebrae differ between the methods, a research study (see reference below for further reading) found a strong to very strong relationship between the X-ray Cobb and Spinal Mouse (Idiag M360) assessments methods (p < 0.0001), and suggest Spinal Mouse (IdiagM360) can be used for research and in the clinical setting as a reliable and easy method, importantly without radiation, for assessing adolescent idiopathic scoliosis.
